This strap needs to be short in order to pull the bra straps in to fit under a racing-back top. However, it is not adjustable at all, and it's just too short for large bras on which the straps are naturally further apart to start with than on a small bra.
If you are over a size 44 bra, don't bother with this, and get the "2004" model also sold here instead. This one is just too short to work with any larger bust.
What is nice about it is that it can be worn in front, too, to pull your bra straps together and prevent them from showing in arm holes or under some tank-top straps.
I will recommend this only to average- to small-busted friends. It's also twice the price it should be for a simple short piece of soft plastic. Solution? They should pack two in each envelope, which would be particularly helpful if you needed to draw in the straps front AND back for, e.g., a tank top with cutaway shoulders.

If I put it on the bra strap twist, needs something to keep the bra stap flat....

I wore this product a couple nights ago with a sleeveless dress. It held up my extra large breasts as well as preventing my bra straps from slipping down or showing on my shoulders beyond the sleeves. It is a great product. I just wish it could be put on before the bra is donned and fastened. I had to have my husband attach it for me.
